This is a collaborative approach to effectively integrate shared health plans that synergistically incorporate the patient's physical and behavioral health care issues and needs holistically. This collaboration of Primary care and behavioral health providers using shared care plans that incorporate patient goals, increases patient engagement, and improved patient outcomes.
