1. Measure your blood sugar levels

Book an initial video appointment with a Circle Medical provider to discuss your health and concerns with prediabetes and/or diabetes.

For existing diabetic patients, your provider will work with you to establish a care plan that works for you and can help refill existing treatments. Your provider will also review the latest treatments or lifestyle modifications that will make living with diabetes more effortless than ever.

If you haven't had a screening test but are at risk of developing prediabetes and/or diabetes, your provider will order blood work for you. Common blood tests include checking your hemoglobin A1C and a metabolic panel to check your fasting sugar levels.

For your convenience, you can have the test performed at a Quest or LabCorp location near you.

2. Discuss your results with a provider

Once we have received your test results, your provider will reach out to you to discuss your results and care options.

  • Completed blood work ordered by Circle Medical is uploaded and shared automatically with you in the Circle Medical app.

3. Partner with your provider to improve your health

If diagnosed with prediabetes or diabetes, your provider will work with you to create a care plan that encompasses healthy lifestyle changes and may include medication based on your needs.

You'll check-in with your provider through video appointments as you:

  • Test your blood sugar levels with a glucometer. Your provider will explain when to self-test and when to have bloodwork done.

  • Upload your results to the Circle Medical app so your provider can review them.

  • Learn more about healthy lifestyle changes you can make. Many of our Circle Medical primary care providers also specialize in lifestyle management, and they can create customized care plans for you tailored toward living a healthier lifestyle.

  • Send your provider a chat whenever you have questions or need additional support. You can message your provider and care team through our Circle Medical App whenever you want guidance.

Who should be screened for diabetes and prediabetes?

According to the American Diabetes Association, you should get a blood work test to be screened for diabetes if any of the following apply:

Everybody over 35

The ADA recommends that all adults over 35 get a blood work test. With normal results, adults over 35 should repeat the test every 3 years.

Adults under 35

If you’re an adult under 35, it is recommended that you get blood testing if you have any of the risk factors for type 2 diabetes or prediabetes, which include:

  • Being overweight

  • Having a parent, brother, or sister with Type 2 diabetes

  • Being physically active less than 3 times a week

  • Ever having gestational diabetes (diabetes during pregnancy) or having given birth to a baby who weighed over 9 pounds

  • Being an African American, Hispanic or Latino, American Indian, or Alaska Native person. Some Pacific Islanders and Asian American people are also at higher risk.

Medication

Medication can include insulin and diabetes pills, such as Metformin, that will help regulate your blood sugar levels.

Blood Tests

Your provider will recommend a combination of self-testing and lab testing specific to your needs.

How do I get a blood test through Circle Medical?

Book an initial appointment with any Circle Medical provider. During that appointment, your provider will get to know you and recommend a specific blood test or multiple tests. They will then order the blood work for you, and you can go to a Quest or LabCorp lab near you to have the test performed. Your results will be automatically uploaded to the Circle Medical app and your provider will be in touch.

How do I share an existing blood test result with Circle Medical?

Once you have booked an appointment with a Circle Medical provider, you will be prompted to download our HIPAA compliant mobile app. Upon signing into your account, you will be able to upload files and images via our in-app messaging system for your provider to review and discuss with you.

What if I just need a diabetes medication refill? Can I get that through Circle Medical?

To get a medication refilled, book an initial appointment with any Circle Medical provider. Medication should be used in combination with diet and lifestyle improvements, and your provider will be able to spend that initial appointment getting to know you and your health history. Any medications will be sent to a pharmacy of your choice.

I haven’t really been managing my prediabetes or diabetes… where should I start?

We understand. Making lifestyle changes can be intimidating, and it can be difficult to stay in touch with your provider for various reasons. Once you book an initial appointment, you’ll be able to discuss your health history with your provider, who will then recommend a care plan that is specific to your needs. You can reach out to your provider at any time through in-app chat, and video appointments are easy to book and join, which makes getting support for diabetes management a little easier.

I’m starting to experience other health concerns. How do I know if it’s related to my diabetes?

It really depends on your individual health, so the best next step for you is to book an initial appointment with any Circle Medical provider. Our primary care providers are able to diagnose and treat all general conditions outside of diabetes care, so we encourage you to find a Circle Medical provider that you trust and can share all of your healthcare concerns with.

How do I share my glucometer results with my provider?

You can use our in-app messaging system to upload files and images for your provider to review.

How often do I need to have appointments?

Your specific appointment frequency depends on your individual care plan. If this is a new diagnosis for you, you may have more frequent appointments with your provider to allow room for modifications to your care plan based on what is working. Patients who have stabilized their blood sugar levels and are focused on maintenance typically see their providers less frequently; about once every 3-6 months. We encourage you to discuss appointment frequency with your provider during your initial appointment if you have questions.

Do I need an in-person appointment for diabetes care?

Our approach to diabetes management is to provide better access to high quality providers through video appointments and in-app chat, so you can get support whenever you need it. Living a healthier lifestyle is a long-term process, and you deserve support that can adapt to your needs.

Do I have to take medications if I am diagnosed with pre-diabetes or diabetes?

Your Circle Medical provider will work with you and provide guidance on lifestyle changes. It is possible to control diabetes and prediabetes through adherence to lifestyle modifications when diagnosed and acted upon early.

Where are Circle Medical’s clinics located? Where can I get care?

Circle Medical has in-person clinics in California and is expanding its in-person presence across the US, but you can get high quality care via telemedicine video appointments in 25 states across the US.

Do I need to book my appointment with a doctor (MD or DO), or can I book my appointment with any provider (PA-C, NP, etc)?

You do not need to book your initial appointment with a doctor. You can book your appointment with any provider that we have available. Our providers all work closely together to make sure that we are providing the same excellent standard of care to all patients regardless of which provider you book your appointment with.

What is the total cost of treatment?

Circle Medical is a primary care practice, which means that the only fee due to us is the cost of the appointment itself. We accept insurance, so your plan may pay some or all of the appointment fee. Circle does not charge any ongoing membership fees to use our platform.

If you are prescribed medications, you will pay for them when you pick them up at a pharmacy.

Insured

We contract with most PPO insurance plans. If you are a California Bay Area resident, we also work with a small number of HMO plans.

To verify if we work with your insurance, simply start booking an appointment online. You will be prompted to enter your insurance details and will receive eligibility results instantly as well as an estimate of your appointment cost prior to confirming your appointment.

Some PPO plans that we currently work with are:

Anthem Blue Cross/Blue Shield, Aetna, Blue Shield of California, Cigna, Health Net, Humana, and United Healthcare.

Our insured patients pay an average of $5-$50 per appointment.

Plans that we cannot work with are:

Unfortunately, we are unable to provide care for patients who have Medi-Cal or Medicaid insurance. This is because we are not licensed Medi-Cal or Medicaid providers, so we are not able to provide care, even if these patients choose to pay cash instead of use their insurance. If you have Medi-Cal or Medicaid, please do not book an appointment with us, as we will be unable to provide care even if you do not enter your insurance information.

Uninsured

We also offer a self-pay option at a flat rate of $100 per appointment.

Your provider will send any prescriptions to your preferred pharmacy, and we will alert you in the app when your prescription is ready for pick-up. This can take up to 24 hours after your appointment.
