Book a video appointment with a Board-Certified primary care provider and get seen today.
Circle Medical has over 200 Board-Certified primary care providers trained in managing diabetes, and a mobile app that makes it easy for you to stay connected.
Book an initial video appointment with a Circle Medical provider to discuss your health and concerns with prediabetes and/or diabetes.
For existing diabetic patients, your provider will work with you to establish a care plan that works for you and can help refill existing treatments. Your provider will also review the latest treatments or lifestyle modifications that will make living with diabetes more effortless than ever.
If you haven't had a screening test but are at risk of developing prediabetes and/or diabetes, your provider will order blood work for you. Common blood tests include checking your hemoglobin A1C and a metabolic panel to check your fasting sugar levels.
For your convenience, you can have the test performed at a Quest or LabCorp location near you.
Once we have received your test results, your provider will reach out to you to discuss your results and care options.
Completed blood work ordered by Circle Medical is uploaded and shared automatically with you in the Circle Medical app.
If diagnosed with prediabetes or diabetes, your provider will work with you to create a care plan that encompasses healthy lifestyle changes and may include medication based on your needs.
You'll check-in with your provider through video appointments as you:
Test your blood sugar levels with a glucometer. Your provider will explain when to self-test and when to have bloodwork done.
Upload your results to the Circle Medical app so your provider can review them.
Learn more about healthy lifestyle changes you can make. Many of our Circle Medical primary care providers also specialize in lifestyle management, and they can create customized care plans for you tailored toward living a healthier lifestyle.
Send your provider a chat whenever you have questions or need additional support. You can message your provider and care team through our Circle Medical App whenever you want guidance.
According to the American Diabetes Association, you should get a blood work test to be screened for diabetes if any of the following apply:
The ADA recommends that all adults over 35 get a blood work test. With normal results, adults over 35 should repeat the test every 3 years.
If you’re an adult under 35, it is recommended that you get blood testing if you have any of the risk factors for type 2 diabetes or prediabetes, which include:
Having a parent, brother, or sister with Type 2 diabetes
Being physically active less than 3 times a week
Ever having gestational diabetes (diabetes during pregnancy) or having given birth to a baby who weighed over 9 pounds
Being an African American, Hispanic or Latino, American Indian, or Alaska Native person. Some Pacific Islanders and Asian American people are also at higher risk.
It is important to understand that the effects of diabetes develop gradually and worsen the longer that your blood sugar is in a dangerous range.
Prediabetes increases your risk of Type 2 diabetes, stroke and heart attack. Type 2 diabetes can commonly lead to heart disease, chronic kidney disease, nerve damage, and other problems with feet, vision, hearing, and mental health.
If you have prediabetes or diabetes, you should seek care because these long-term effects will eventually become disabling and even life-threatening.
You can take steps to prevent serious health complications of prediabetes and diabetes by bringing your blood sugar back down to healthy levels and improving your overall health.
Finding a primary care provider you trust is key to figuring out how to make and maintain long-term lifestyle changes that will keep you healthy and happy.

Based on your initial appointment, your provider will suggest strategies for living a healthier lifestyle.
You'll then work with your provider to figure out what does or doesn't work for you, and your provider will adjust your care plan appropriately to make sure it suits your needs.
Your provider may recommend medication to supplement your treatment plan based on your needs.
Medication can include insulin and diabetes pills, such as Metformin, that will help regulate your blood sugar levels.
Your provider will recommend a combination of self-testing and lab testing specific to your needs.
For self-testing, your provider will explain when and how often to take your blood sugar levels, which you can upload to the app to share with your provider.
For lab tests, your provider will order your blood work, which you can get done at a lab near you.

We are trusted by over 50,000 patients every month. Here's what they have to say.
This app met me on my level for my neurodivergent needs, didn't keep me waiting, and has really kind and professional medical staff.
My provider is amazing. So trauma-informed and willing to listen. She was able to support me in ways so many others doctors had not over the years simply by listening to my concerns and all the ways I had tried to treat my condition on my own. This practice is excellent and I'm so grateful to have found them. I highly recommend Circle Medical.
I get to decide my provider. Tons of appointments that are days away instead of weeks. Well-designed app. Medical/appointment notes are all in one location instead of spread around or buried. Everything got done without any bumps or pushback or bs. I've never *ever* had such a painless experience.
Always prompt to reply, very friendly and knowledgeable.
Accessibe, efficient, and I really like my provider's approach to my concerns and partnership in treatment planning. I feel validated, heard, and respected. Love how streamlined the app makes everything including the chat feature being open 24/7. Feels like I have open/quick access and flexibility in getting my needs met.
My provider is honestly the best I have ever had the pleasure of seeing. Her “bedside manner” and the amount of care and concern she has for her patients (and patients as real humans, not just numbers) is beyond compare.
Very understanding and eager to help with you mental and physical wellbeing.
It’s very easy to use and my practitioner is very attentive, responsive, and easy to talk to.
I love how well-informed and empathetic my PCP is. It has been the best psychiatric provider experience I have had thus far in my life, I trust her so much more than my previous providers.
It was such an easy process, my provider may be one of the best I’ve ever had in my life
Easy to book an appointment, huge selection of days and times, you can use different providers. Everyone is helpful and respectful.
Easy to schedule appointments, the chat is super responsive, and everyone I’ve talked to is friendly and helpful.
Excellent providers that truly care about their patients and listen to their concerns. Telehealth appointments are seamless.
Great selection of providers. They are kind and knowledgeable. Easy process - insurance accepted! Did my check in while at work, doesn’t get better than that!
Great group of physicians. They have been great and quick to answer. I love the fact that in todays world we can do this on the phone. It’s convenient for everyone
Everything works so smoothly! Right from home or work. You don’t have to worry about being late because of traffic.
Convenient, effective, and efficient heath care provider. It’s the modern way to get heath care.
The efficiency of not having to go to an office and the flexible hours are great!
Because the doctors are very professional and also they have compassion unlike others in the past
Awesome service, many appointment times available, caring and thorough medical care. Perfect for people with busy schedules.
I like the flexibility of virtual appointments.
Always easy to get a convenient appointment. Very accurate. This is the future of healthcare.
Scheduling was easy. Appointments started on time. I needed help with some preauthorization stuff for a prescription and the response time was quick and efficient via chat.
I love not having to wait before my appointment begins in the lobby and not having to drive there also saves time. It was also great when I had COVID, as I did not have to expose anyone and still could get medical help. I also got life changing diagnosis of ADHD through Circle Medical. I LOVE MY PROVIDER, she is amazing!
Easy process, excellent providers and a phenomenal app experience.
Quick, kind and professional in communicating with me.
The providers have been exceptionally responsive and giving me great advice. I can get an appointment often within hours if I need something urgently.
I love having a primary care provider to work with me on getting the correct medication needed to function. I love the convenience of the telehealth appointments as well.
Comfortable experience, and from my home.
Very professional and non-judgmental. System is very easy to use.
Circle Medicine gave me access to care that if I had not been able to access through telemedicine I most likely would not have gotten it. Not to mention the ease of use, which is an absolute 10. Technology at its finest.
Honestly my provider has been vastly more concerned about my needs than any doctor I've been too before, and I don't even have to leave my house. This has been an absolutely amazing medical experience.
The service is great and flexible and I love my provider! She is amazing. She listened to what I have to say and all my concerns that have been dismissed in the past with other doctors. She never makes me feel silly for my concerns and lets me vent about why I was concerned about it.
My provider has been nothing short of amazing with helping me with ADHD diagnosis and treatment.
Ease and simplicity of signing up, scheduling, communicating, and not having to waste time going to an office. Saves hours and hours of time per year.
I was put together with a great provider. She is knowledgeable and patient and honestly she has better bedside manner than many of the therapists I have seen. The chat feature has been very useful. Response time is faster than many other medical services. I have enjoyed my experience with Circle Medical! I do not miss my old provider at all! Thank you.
Fantastic doctors, NPs, communication. Ease of use, virtual appointments, on time for every appointment. They are quick and thorough.
My provider was great! Very friendly and made me feel comfortable talking about some uncomfortable parts of my life.
Easy to get appointments, provider is kind, very helpful and is committed to helping me. This app was there when my doctors ignored me for YEARS.
My interaction was one with care, and I was asked more questions into my “issues or problems.” It didn’t feel like I was talking to a typical doctor, but rather someone who was genuinely concerned with my problem.

Book an initial appointment with any Circle Medical provider. During that appointment, your provider will get to know you and recommend a specific blood test or multiple tests. They will then order the blood work for you, and you can go to a Quest or LabCorp lab near you to have the test performed. Your results will be automatically uploaded to the Circle Medical app and your provider will be in touch.
Once you have booked an appointment with a Circle Medical provider, you will be prompted to download our HIPAA compliant mobile app. Upon signing into your account, you will be able to upload files and images via our in-app messaging system for your provider to review and discuss with you.
To get a medication refilled, book an initial appointment with any Circle Medical provider. Medication should be used in combination with diet and lifestyle improvements, and your provider will be able to spend that initial appointment getting to know you and your health history. Any medications will be sent to a pharmacy of your choice.
We understand. Making lifestyle changes can be intimidating, and it can be difficult to stay in touch with your provider for various reasons. Once you book an initial appointment, you’ll be able to discuss your health history with your provider, who will then recommend a care plan that is specific to your needs. You can reach out to your provider at any time through in-app chat, and video appointments are easy to book and join, which makes getting support for diabetes management a little easier.
It really depends on your individual health, so the best next step for you is to book an initial appointment with any Circle Medical provider. Our primary care providers are able to diagnose and treat all general conditions outside of diabetes care, so we encourage you to find a Circle Medical provider that you trust and can share all of your healthcare concerns with.
If you are experiencing severe health concerns, such as [ ], please dial 911 or visit your nearest emergency room.
You can use our in-app messaging system to upload files and images for your provider to review.
Your specific appointment frequency depends on your individual care plan. If this is a new diagnosis for you, you may have more frequent appointments with your provider to allow room for modifications to your care plan based on what is working. Patients who have stabilized their blood sugar levels and are focused on maintenance typically see their providers less frequently; about once every 3-6 months. We encourage you to discuss appointment frequency with your provider during your initial appointment if you have questions.
Our approach to diabetes management is to provide better access to high quality providers through video appointments and in-app chat, so you can get support whenever you need it. Living a healthier lifestyle is a long-term process, and you deserve support that can adapt to your needs.
Your Circle Medical provider will work with you and provide guidance on lifestyle changes. It is possible to control diabetes and prediabetes through adherence to lifestyle modifications when diagnosed and acted upon early.

Our providers all work closely together to make sure that we are providing the same excellent standard of care to all patients regardless of which provider you book your appointment with.
Circle Medical is a primary care practice, which means that the only fee due to us is the cost of the appointment itself. We accept insurance, so your plan may pay some or all of the appointment fee. Circle does not charge any ongoing membership fees to use our platform.
If you are prescribed medications, you will pay for them when you pick them up at a pharmacy.

Our insured patients pay an average of $5-$50 per appointment.
Plans that we cannot work with are:
Unfortunately, we are unable to provide care for patients who have Medi-Cal or Medicaid insurance. This is because we are not licensed Medi-Cal or Medicaid providers, so we are not able to provide care, even if these patients choose to pay cash instead of use their insurance. If you have Medi-Cal or Medicaid, please do not book an appointment with us, as we will be unable to provide care even if you do not enter your insurance information.
We also offer a self-pay option at a flat rate of $100 per appointment.
Please take a minute to review our FAQ and make sure that you are a good candidate for treatment through our practice before you book your appointment.

Download the app right after you book your appointment, and you’ll receive a few questionnaires to complete prior to your visit.
A few minutes before your scheduled appointment time, you will receive a link in the in-app chat to join the video call. Make sure that you are in an area with a strong cellular connection.
If you are a good candidate for care, your provider will then discuss your treatment plan. Again, be sure to read through the FAQ to make sure that you are a good candidate for care before booking your appointment.
Your provider will send any prescriptions to your preferred pharmacy, and we will alert you in the app when your prescription is ready for pick-up. This can take up to 24 hours after your appointment.
